Registration for Difficult Dogs
You can either print out a Microsoft word form or complete the on-line registration.

A complete registration includes: form, payment, copies of vaccinations and city license.  Note: if your dog is younger then 4 months old - waive the city license.

Step Six - Participation Agreement and Waiver

I/We      (hereinafter referred to as “Client”) have voluntarily employed K-9 Partnership (hereinafter referred to as “Consultant”)  to assist me in the education and training of my dog(s)   

 The success of companion pet programs depends on a combination of learned skills on the part of the pet and the owner. Behavior is not static; an animal will not continue to perform even trained behaviors without ongoing practice. The pet’s behavior is ultimately the client’s responsibility.  

I hereby agree by the rules and policies set forth by K-9 Partnership. I have read and understand the policies.  I understand that attendance to this class is not without risk to myself, members of my family, guests who may attend, or to my dog because some of the dogs to which I will be exposed to may be difficult to control and may be the cause of injury even when handled with the greatest amount of care. I am voluntarily participating in these activities with knowledge of the risks involved, and hereby agree to accept any and all risk of loss, damage, injury, disability and/or death at any time to myself or my dog.

In the acceptance of your application for dog education and training classes provided by K-9 Partnership, I agree that I will not sue, or otherwise make any claims against K-9 Partnership for injury, loss or damages resulting from the negligence or other acts, however caused by the consultant as a result of dog bites and/ or my participation in physical activities involved in training or observing in such activities. I hereby agree to indemnify and hold harmless K-9 Partnership, its officers, directors, coaches, agents, volunteers and/or representatives from any and all claims by any member of my family or any other person accompanying me to any training session or function or while on the grounds or the surrounding area thereto as a result of any action by any dog, including my own.  I will also hold harmless K-9 Partnership should an accident or injury occur prior to, during or following the dog education and training classes. In addition, I agree that I will defend and indemnify K-9 Partnership for any injury, expense, costs or damages to any dog handlers or dogs, whether sponsored by me or not, or to third parties arising out of my actions or the actions of my dog.  I have read the above-stated provisions, and agree to accept those responsibilities.

 By checking this document, I also hereby certify  that my dog(s) does not have any contagious diseases and I warrant that I am  in good health and have no physical condition that would prevent me from participating in these dog education and training classes.

I the undersigned, acknowledge that I have read the foregoing paragraphs, and that I am fully aware of the potential dangers and risks incidental to participating in this training. I understand and accept to my responsibilities written herein, and am fully aware of the legal consequences of accepting this written instrument and am doing so of my own free will.

 I have read and understand K-9 Partnership’s Policies and Participation Agreement/Waiver.
